الكفاءة السيكومترية لمقياس يونج لتقدير شدة أعراض الهوس في الثقافة المصرية

الملخص
    هدفت الدراسة الحالية إلى فحص صدق وثبات ودقة تشخيص مقياس يونج لتقدير شدة أعراض الهوس الهوس Young Mania Rating Scale (YMRS) في البيئة المصرية، على عينة من مرضى اضطراب ثنائي القطب في نوبة الهوس ونوبة الاكتئاب والأسوياء، واشتملت عينة الدراسة على 67 فردًا (34 ذكر، 33 أنثى)، وتراوحت أعمارهم من(17: 56) عامًا بمتوسط =29.48، وانحراف معياري = 9.04. وتكونت مجموعة المرضى من (35) مريضًا باضطراب ثنائي القطب، و(32) من الأسوياء، واشتملت أدوات الدراسة على مقياس يونج لتقدير الهوس ومقياس هاملتون للاكتئاب، والمقابلة الإكلينيكية لتشخيص الاضطرابات لدى الراشدين نسخة عربية مستمدة من الدليل التشخيصي الرابع DSM-4، بالإضافة إلى البيانات الأساسية والمسح الشامل للأعراض، وقائمة أعراض اضطراب ثنائي وفقًا للدليل التشخيصي والإحصائي الخامس للاضطرابات العقلية .DSM-5 وأظهرت النتائج أن مقياس يونج لتقدير الهوس قد حقق درجة مرتفعة من الصدق والثبات، فقد أظهر اتساقًا داخليًا لجميع البنود مع الدرجة الكلية للمقياس؛ فقد تراوحت قيم معامل ارتباط بيرسون (من 0.502 إلى 0.767)، مما يشير إلى وجود ارتباط طردي دال إحصائيًا لجميع البنود مع الدرجة الكلية للمقياس، كما أظهر ثباتًا بمعامل ألفا كرونباخ قيمته (0.814) وبطريقة التجزئة النصفية بقيمة (0.751)، كما حقق ثباتًا بطريقة إعادة التطبيق بين استجابات العينة السيكومترية على عبارات المقياس خلال التطبيق وإعادة التطبيق ارتباط دال إحصائيًا أكبر من 0.7 مما يشير إلى تحقق ثبات المقياس بطريقة إعادة التطبيق، وفيما يتعلق بصدق المقياس فقد أظهر الصدق التلازمي للمقياس مع المحك الخارجي وهو محكات تشخيص الهوس في الدليل التشخيصي والإحصائي للاضطرابات العقلية، الإصدار الخامس (DSM5) معامل الارتباط دال إحصائيًا بين الدرجة الكلية لمقياس يونج والدرجة الكلية لمحكات تشخيص الهوس في (DSM5) بلغت قيمة الارتباط (0.896)، مما يشير إلى تحقق صدق المقياس.
Psychometric Efficiency of the Young
Mania Rating Scale as a Mania Severity Measure. in Egyptian Culture.
Abstract
The present study aims to assess the reliability, validity and Diagnostic accuracy of- the Young Mania Rating Scale (YMRS)-. in Egyptian Culture. Based on a sample of bipolar disorder patients in a manic episode, a depressive episode, and healthy individuals, the sample include (67) individuals (34 males, 33 females), whose ages range from (17: 56) years, with an average =29.48, and standard deviation=9.04. The patient group includes (35) patients with bipolar disorder as well as (32) healthy individuals. In addition to baseline data and comprehensive symptom survey and a list of symptoms of bipolar disorder according to the fifth edition of Diagnostic and Statistical Manual of Mental Disorders (DSM-5), materials and method are: the Young Mania Rating Scale (YMRS), Hamilton Depression Rating Scale (HDRS), The Clinical Interview for Diagnosing Disorders in Adults, an Arabic version, derived from the Fourth edition of Diagnostic and Statistical Manual of Mental Disorders )DSM-4(.The results have showed that the Young Mania Scale has achieved a high degree of validity and reliability. It has showed internal consistency for all items with the total score of the scale; the Pearson correlation coefficient values have ranged from (0.502: 0.767), and this, in turn, indicates that there is a statistically significant direct correlation between all items and the total score of the scale. Additionally, it has showed reliability with Cronbach's alpha coefficient of (0.814), as well as half-split method of (0.751). Moreover, it has achieved reliability by the re-test method, between the psychometric sample responses and the scale statements during the test and re-test have showed a statistically significant correlation greater than )0.7(. As for the validity of the scale, the correlative validity of the scale with the external test, which is a criterion of diagnosing mania according to the Diagnostic and Statistical Manual of Mental Disorders Fifth Edition (DSM5), has presented the correlation coefficient as a statistical significant factor between the total grade in Young Scale and the total grade of the criteria of mania diagnosis. The correlation value has been (0.896) and this refers to the validity of the scale.
